Patient Forms

Below are the forms you will complete at your MRI appointment.  As a convenience, we have provided these forms in advance so that you can prepare for your safety screening and scan.  In order to bill your insurance, you must bring a copy of your card to the appointment along with your drivers license and films from prior MRI or mammograms.  You may complete these forms prior to your appointment, but this is not necessary.  If you have any questions, please call 810-230-2411, or email contactus@greaterflintmri.com
